About the Role

We are looking for an experienced Care Home Assistant Practitioner (CHAP) to join our team in Evenwood.

Working alongside our Registered Nurses, you will provide high-quality, person-centred care and support with a range of clinical tasks. This is a great opportunity for someone who is CHAP trained, confident in their skills and passionate about supporting people with complex care needs.


What You'll Be Doing

  • Supporting Registered Nurses to provide safe, high-quality care
  • Carrying out clinical tasks once trained and assessed as competent
  • Supporting with medication, wound care and catheter care
  • Completing observations including blood sugar monitoring and vital signs
  • Supporting with PEG/PEJ care and other complex care needs
  • Providing personal care and supporting residents with mobility and daily activities
  • Maintaining accurate care records and documentation
  • Promoting dignity, independence and choice
  • Supporting care staff through guidance, mentoring and supervision
  • Contributing to care plans, risk assessments and health & safety


About You

We're looking for someone who has:

  • Completed an accredited Care Home Assistant Practitioner (CHAP) programme
  • NVQ Level 3 in Health & Social Care (or equivalent)
  • At least 12 months' experience in a similar healthcare role
  • Experience with medication and personal care
  • Experience supporting people with complex care needs
  • A good understanding of MCA and DoLS
  • The ability to work confidently as part of a wider care and nursing team

Desirable:

  • Phlebotomy experience
  • Experience supporting people with disabilities or complex needs
  • Previous experience supervising or supporting a team
